chet suggests that he can do a one stage SRS procedure using a scrotal skin graft. however, if you haven't got enough donor material he does take an abdominal skin graft at extra cost (his website says he takes this in 10% of cases)! this leaves a scar right across your stomach around your bikini line and also means he might not be as cheap as he initally sounds.
his website says he aims for at least five inches depth, with an avarage of six inches.
his prices are

both with epidural blocks as well as general anesthesia. apparantly the heaviest post-op pain management is limited to tramadol 'cause they don't like to dish out morphine. the hospital looks nice and shiny though.
he wants a 10% deposit wired to his account before he confirms the date, but it seems that he confirms and holds dates even before you've done this (i took about three weeks from getting my date to even getting back in touch with him and it still held).
he also wants a bunch of tests done 1 month before he'll work on you. these are:
1. CBC (complete blood count and platelet count)
2. Anti HIV (uh, i guess he means HIV antibodies?)
3. BUN & Cr (kidney function test)
4. PT & PTT (screening test for bleeding tendancy due to liver disease)
5. FBS (fasting blood sugar)
6. Chest x-ray
and if you're over 45 he wants an EKG.
they will apparantly do these tests there for $150usd, which may or may not be cheaper than you can get them at home. i don't know. i'm british. we still almost have something approaching free healthcare. ha.
you get about five days in the hospital and once your packing, drains and catheter are out and you've proved you can still piss they send you back to the hotel. you get daily visits by nurses, but they expect you stay in the hotel for two weeks after discharge. there are two hotels, but the baan siriami is the cheapest at about $22-25USD a night. this doesn't include breakfast. cheapskates. 'course, add this up over 20 nights and it gets pretty fucking expensive. but, the rooms are twin ones so if you can somehow end up sharing them with somebody you really don't mind dilating in front of for a fortnight solid it'll bring the cost down.
overall, so far i've only dealt with the guy on e-mail, but he seems relatively straightforward. he has booked the hotel for me, will be sending people and a driver to the airport, and has been really simple to deal with over the whole SRS Letter thing. he says on his website that he needs signed origionals, but he's told me that he's happy with photocopies.
oh yeah, and he apparantly also sells dilators for $100. they come in a colourful silk bag. again, i can't tell you if this is cheaper than other alternatives or even if there is a DIY method of making dilators.
the dilation regime requires you to spend 30 to 40 minutes dilating three times a day.
otherwise, things i have heard: the anesthetist is hot.
sensitivity is his speciality.
the labia majora he constructs don't look particularly big.
